Cardiopulmonary Rehabilitation Services
How we help you and your patients
- Patient convenience. Rehabilitation provided closer to the patient’s home. Transportation is provided as needed. Parking is located across the street and visible from the facility.
- Individualized attention. JMH Rehab limits the number of monitored patients to eight per session, which provides a more comfortable and secure environment.
- Follow up on patient progress. Patient progress notes are provided in accordance with your preferences.
- Building patient confidence. Routine integration of Phase III patients provides a support group aspect to Phase II patients. An active support group—Healing Hearts—contains over 40 members.
Quality/Performance Indicators
- The rate of patients who demonstrated improvement in functional capacity for the twelve—month period ending September ’03 is 88 percent.
- The rate of patients who demonstrated a 10 percent improvement in their feeling of well-being was 100 percent for the twelve month period ending September 2003.
