Home Care Team
You will quickly discover that our staff are caring, committed, and qualified professionals. They are members of the community just like you, and they take great pride in providing each patient with personal, compassionate care. And, with health care constantly changing, you can rest assured that our staff receive ongoing education and training so they can provide you with the latest in quality care. Home care staff members include:
- Skilled Nursing - a Registered Nurse will provide an assessment of your individual needs and, after consultation with your primary care doctor, will implement a plan designed specifically for you.
- Physical Therapist - a Physical Therapist will provide therapeutic assistance with exercise, gait training, and transfer training. They can also set up a maintenance program for you and your family to follow.
- Occupational Therapist - an Occupational Therapist will assist you with becoming as independent and productive as possible in the performance of tasks that make up your daily life. Programs include activities designed specifically to develop or increase those skills ... and, as needed, techniques or equipment that can replace absent skills or abilities.
- Speech Therapist - a Speech Therapist will provide you with individualized care dealing with speech or swallowing difficulties. The therapist will work with you and your family in establishing an appropriate communication style that all parties understand and are comfortable with.
- Medical Social Worker - a Medical Social Worker will come to your home and help you and your family work through any financial, resource, or counseling issues you may be facing. The Social Worker is very knowledgeable regarding what community services are available, and they use that information to your benefit. They can help you deal with filling out Medicare or Medicaid applications, finding additional resources for transportation, locating programs that provide personal care options, and assisting with nursing or boarding home placement.
- Personal Care - Home Health Aides will provide you with personal care in your own home. They can perform light housekeeping duties, assist with bathing, and prepare light meals or snacks. As Home Care is a team effort, aides work in conjunction with other services such as Skilled Nursing or Physical Therapy to provide you with the best possible coordinated care.
