Hospice care is provided by an interdisciplinary team of professionals in partnership with the patient's physician. The interdisciplinary team includes:

  • Our Medical Director is available to consult with your primary physician and may assist or manage your medical care if your physician requests, or is unavailable.
  • Nursing Staff, highly skilled in pain management, symptom control, while providing guidance and supportive care.
  • Social Workers — Social Workers help relieve the emotional and social pain of patients and their families, address family financial concerns, and assist with coordination of community resources and funeral pre-arrangement.
  • Spiritual Counselors — Spiritual Counselors offer spiritual guidance and emotional support to patients and their families.
  • Volunteers - Volunteers assist with various needs: sitting with patients, running errands, providing relief for family members, or assisting in community outreach. These special individuals offer friendship and emotional support that complement hospice's total care program.
  • Home Health Aides — Home Health Aides provide personal care, grooming, bathing, skin care, and homemaking.
  • Physical, Occupational, and Speech Therapists - Therapists provide palliative modalities to enhance quality of life for Hospice patients.
  • Dietary Counselors — Dietary Counselors are available to assess the nutritional needs of patients.
  • Bereavement Services - Includes grief education, memorial services, support groups, and ongoing support by caring, experienced counselors. Bereavement Services are provided for 12 months, or as long as needed, at no cost.
