Wound Care Center Patient Information
Perhaps you or a loved one has a sore or wound that hasn't started to get significantly better in a month, or healed entirely in two months. If so, you may benefit from the Wound Care Center at Jefferson Memorial Hospital.
A physician's referral is not required, though we will work closely with your doctor and provide regular written updates on your condition.
Wound Care at Jefferson Memorial Hospital offers you a comprehensive outpatient approach at a single location that can result in earlier recovery, reduced hospitalization, prevention of prolonged or permanent disability, and a high healing rate for complicated wounds.
A Wound Care program that complements your physician's care.
Your personal physician will continue to treat any underlying conditions and manage overall patient care. The wound care team will treat the chronic or non-healing wound and remain in contact with your primary physician during the course of treatment.
How can the Wound Care Center help?
The Wound Care Center is dedicated to chronic non-healing wound management and provides a thorough diagnostic appraisal, followed by a carefully designed case management program. Our multidisciplinary team provides wound assessment, treatment, and caregiver education. Non-invasive tests and a variety of treatments are available to facilitate healing.
The most common sufferers of chronic wounds are individuals with:
- Pressure ulcers
- Diabetes
- Venous insufficiency
- Arterial insufficiency
- Post-surgical wounds
- Traumatic wounds
How will my wound care treatment begin?
On your first visit, our staff will evaluate your general health and your wound. Your medical history will be reviewed as well. A blood test may be involved. You also may need special tests that give information about blood flow and oxygen flow in the wound area.
How will my treatment plan be developed?
Once your test results are received, we'll develop a treatment program based on your special needs. This program will include regular visits to the Wound Care Center to provide treatment, evaluate progress and make any modifications as needed. You or your caregiver will be given detailed instructions in home care, dressing changes and protecting the wound from further injury.
How will my physician remain part of the care team?
The Wound Care Center works closely with your personal physician. We'll discuss your treatment program with your doctor and keep him or her updated on your progress, unless instructed otherwise. You should continue to see your physician for routine medical care.
What expectations can I have?
You are a vital part of the program. Much of the success of your treatment depends on you. You must keep your appointments, follow directions carefully and watch your progress closely in between visits. Any time you, a family member, or caregiver has questions, the Wound Care team is available to help.
